This report does not in itself initiate a formal complaints process, however you may request this and explore this option with a Case Manager. In certain circumstances, The Open University may judge that action is required even if the person reporting is not requesting any. This will be done in accordance with the University staff and student
Dignity and Respect Policy and staff
Bullying and Harassment Policy.

At the end of this form, we will ask some questions about you which are
optional and can be skipped. This is so that we can monitor the different experiences of certain groups and be able to take more focused action. Completing these questions will help us analyse results accurately. All results will be published in an anonymised way and raw data will not be provided for counts less than 5.
The information contained in this report will only be passed on to relevant staff members on a strictly need to know basis and in line with our
privacy notice. The Open University will not disclose any personal or identifiable information to others, except as set out in the
Safeguarding Policy.
